Thank you very much for adopting your puppy from Dreams & Tails Kennel, LLC.  The puppy you have adopted
from Dreams & Tails is guaranteed to be healthy at the time of leaving our kennel.  This health guarantee
stipulates that you take the puppy to a licensed veterinarian
within 72 hours-three days- of puppy leaving the
kennel; failure to do so will nullify this health guarantee.  The puppy will be current on all age appropriate
vaccinations and de-worming.  Dreams & Tails guarantees the puppy for ten (10) days from the date of adoption
upon leaving kennel premises against Parvo virus, Distemper and Parainfluenza.  This guarantee does not cover
minor illnesses, hernias, hypoglycemia, illnesses caused by the ingestion of foreign objects or chemicals, or
physical injury.
 THIS CONTRACT IS VOID UNLESS WE HAVE RECEIVED FROM YOU THE ORIGINAL COPY OF THE
HEALTH AGREEMENT SIGNED AND DATED BY YOU AND YOUR VET WITHIN 10 DAYS OF RECEIVING PUPPY.  Should
your puppy contract any of these diseases, Parvo virus, Distemper or Parainfluenza, within this period, Dreams
and Tails Kennel should be contacted at once and requires a signed form with the diagnosis from your licensed
veterinarian.  Dreams & Tails reserves the right to request an examination by our own veterinarian, and to ask
that our veterinarian be contacted by the consulting veterinarian, or one recommended by our veterinarian.

Dreams and Tails also guarantees the puppy against LIFE THREATENING hereditary defects only for a period of
one year from the puppy's date of birth.  In the event of such occurrence, Dreams and Tails will replace the puppy
with a puppy of the same gender, quality and value as soon as a puppy is available, provided the veterinarian's
statement is submitted to Dreams & Tails within ten (10) days of the date of the diagnosis.  Should death occur,
Dreams & Tails reserves the right to request the puppy be examined by Dreams & Tails' veterinarian, or a
veterinarian recommended by Dreams & Tails' veterinarian.  If the kennel's veterinarian deems appropriate and
feels it necessary, it may be requested that the deceased puppy's body be returned to Dreams & Tails for the
purpose of a necropsy by the Kennel's own veterinarian.

This puppy is being adopted to a pet home. It is hereby understood by both buyer and seller that the buyer is
responsible for any and all veterinarian costs after the date of adoption and that no other guarantees are made or
implied by Dreams and Tails, except those stated in this written guarantee.  The buyer agrees that Dreams and
Tails' obligation ceases and this contract is void if buyer does not continue a regular veterinary maintenance
program of de-worming, vaccination series, proper care of your puppy, fails to see a qualified veterinarian within
three (3) days after puppy has left kennel; deprives the puppy of any medical treatment needed; or physically
abuses and / or neglects the puppy in any way; or does not follow proper animal husbandry practices.  A signed
(by your licensed veterinarian and new owner) and dated copy of your health guarantee must be returned to
Dreams & Tails to enforce your guarantee within 10 days after adoption.  The new owner and their licensed
veterinarian must sign and date the guarantee within the three (3) day time frame of delivery of puppy in order for
the guarantee to be valid, failure to do so will void this guarantee.  This guarantee does not cover injury, illness,
or death caused from accidents or neglect, minor illnesses, hernias, hypoglycemia, illnesses caused by the
ingestion of foreign objects or chemicals, or physical injury. It is the responsibility of the buyer to provide a safe
and healthy environment for the puppy.  We recommend to avoid letting your puppy jump from or up on sofas, etc,
due to possible hip/leg injuries.  This puppy has been extensively checked by a licensed veterinarian and given a
clean bill of health and sturdiness.  We also highly recommend you continue the program of NuVet vitamins for
your puppy.  We also advise to give your puppy BOTTLED WATER the first couple days then gradually add the
water you normally use for drinking.  If changing food, please mix with the food your puppy has been eating and
change gradually.  

You have been provided with a complete vaccination record with the additional records that came with your
puppy.  Please submit the vaccination record to your veterinarian in order for them to determine an immunization
and de-worming schedule appropriate for your puppy.
